We kindly request that all cancellations be made within 4 hours of your appointment.

- EVERY patient must fill out the PATIENT INTAKE
- If this is related to a car accident, then you will also fill out the CAR ACCIDENT ADDENDUM
- If this is related to a work accident, then you will also fill out the WORK COMP ADDENDUM
- If you are being seen for fertility treatment, then you will also fill out the FERTILITY ADDENDUM
